The first pillar of TPOHC is the initial assessment. With all personal health & fitness coaching, it’s critical to start with an assessment. The assessment is important to determine your starting point and to screen for issues.

If your goal is where you want to go, then the assessment is how we determine where you’re starting from. Once I know your starting point and your goal, I can develop an individualized program tailored specifically to you. Working as a coach since 2007, I've developed an incredibly thorough assessment system for my new clients.

For online coaching, the assessment matters even more; as I won’t be with you in-person. When I created Transformation Possible Online Health Coaching, I decided to make the best Assessment I could. I started with my in-person assessment then I thought to make it even better than in-person. Faster and with more involved. Everything I think of for my own health & fitness work, but in a even easier for you to submit a response.

Over 250 questions!

There’s a LOT of questions in the TPOHC Initial Assessment but it’s worth it. I assess your physical abilities and limitations, as well as social support, readiness to change, kitchen 'health.' I also ask questions about the equipment/facilities you have access to tailor-fit your plan.

None of these questions are useless 'fluff. — every question is included for a reason, and every response is valued.

With this thorough Assessment, I will know the details of your health history, fitness ability & level, goals, and challenges. With that info I can create a safe, effective, and time-efficient individualized training program for you.

Details of the Initial Assessment

I use Google Forms to make it easy for you to submit your responses. And I provide a Welcome Guide with thorough instructions for your submission.

1. Equipment Available Form

I'll be asking you what fitness training equipment, gadgets, and tools you have access to. Pictures all along to make it easy. This form only takes minutes.

2. Digital Physical Assessment form

This portion of the Initial Assessment consists of pictures, measurements, and exercise capacity tests. This is how we determine your starting point.

Pictures allow me to screen for imbalances and to check posture; and may be your 'before' pictures. Next, there are body measurements. We will do body weight, body fat percentage, and/or tape measurements. Take as many or as few of these as you like, but I encourage you to do them all. Every client gets individualized tracking recommendations.

Finally, there are Exercise Capacity Tests. We will choose several tests from five movement categories and record the stats. There are bodyweight movement testing and cardiovascular tests, as well as a baseline workout. Also, strength lifts for advanced trainees. I highly recommend you complete the Baseline workout, but it's okay to skip the strength testing.

3. Intake Form

The final step in the Initial Assessment is the Intake Form. This form is a comprehensive form that will ask many questions related to your health & fitness, habits, lifestyle, and routines. This form is lengthy, but remember, these are crucial steps for creating the best program for you and you only have to do this once at the start of your coaching with me.

After you complete the Intake form, your responses will be complied into a tidy package for me to form the base of your program. I’m serious about EVERY question! The new client packet is over 15 pages and has all your responses formatted for maximum coaching effectiveness. If you can’t tell, I’m fiercely proud of this Initial Assessment. This is how I do a great job for you and it feels extra good knowing I built this entire process myself!
